Care Planning
In collaboration with NHS Sheffield CCG, we are working with some patients who have health conditions to develop person centred care plans. These care plans are designed to help patients better understand their health condition, self manage their health, and support them to reach goals regarding their health. We are also working with other care provides such as District Nurses and community support workers to coordinate how we care for patients and support them with their care.
If you have been invited to attend for a care planning appointment,we will ask you to complete a questionnaire about your health and your approach to your care. It is important you complete this and return it to us before your appointment as we use the information to help us tailor your care plan for you. At your appointment you will have time to discuss your health and care needs and we will work with you to set some goals regarding your health. We will then support you in how you can achieve those goals and better self manage your own care. We may also refer you on to other care providers who can support you. At the end of the appointment you will be given a print out of your care plan which will detail your goals and how you can achieve them. It will also contain details of what to do if you are ill or when things go wrong. Please share your care plan with your family and carers so they can help you achieve your goals. It will also help other care professionals know how to help and support you.
After a few months we will ask you to complete the same questionnaire we sent you at the beginning. Please complete this and return it to us as it helps us see how you are progressing and lets us know if we need to support you in any other areas. We will also contact you for a review of you care plan to see how you are getting on with your goals.
